What is Ilizarov Ring Fixator?
The Ilizarov method, developed by Russian surgeon Gavriil Ilizarov, uses a circular ring frame and tensioned olive wires to stabilise bone while simultaneously allowing controlled gradual correction of deformity, lengthening of short limbs, and regeneration of new bone through distraction osteogenesis. The key biological principle is that if bone is slowly pulled apart at a rate of 1 mm per day (in 4 increments of 0.25 mm), new bone forms in the gap — a process called distraction osteogenesis. This allows the lengthening of limbs by several centimetres, correction of angular and rotational deformities, and regeneration of bone segments lost to infection or trauma.
Suitable for limb length discrepancy requiring correction of > 2 cm, complex angular and rotational deformity, infected non-unions where internal implants are contraindicated, congenital deformities of the lower limb, and tibial plateau or periarticular fractures requiring fine deformity control.
How the Procedure Works
Pre-operative Planning
CORA (Centre of Rotation of Angulation) analysis and full deformity planning determine the frame configuration and correction programme.
Corticotomy (for lengthening/correction)
A low-energy bone cut (corticotomy) is performed through a small incision — preserving periosteum and medullary blood supply, maximising regenerate quality.
Wire Insertion & Frame Assembly
Tensioned olive wires are passed through the bone at multiple levels; rings are assembled around the limb and wires are tensioned to 110–130 kg.
Distraction Programme
After a 7-day latency period, distraction begins at 1 mm/day (4 x 0.25 mm turns); the patient adjusts the frame at home.
Consolidation & Frame Removal
Once the desired correction is achieved, the frame is retained until regenerate bone consolidates (typically 2x distraction time); then removed in clinic.
