✦ Fracture Fixation

External Fixation of Fractures

Pins inserted through the skin into bone are connected to an external frame — the preferred fixation for open fractures, polytrauma damage control, and complex periarticular injuries requiring staged management.

What is External Fixation of Fractures?

External fixation stabilises a fracture using metal pins or wires inserted percutaneously through the skin and bone on either side of the fracture, connected to a modular frame outside the body. This achieves reliable fracture stabilisation without internal implants — making it the method of choice when infection risk, soft tissue damage, or fracture complexity makes internal fixation inappropriate. External fixation includes simple uniplanar frames for straightforward shaft fractures, the Ilizarov ring fixator for complex deformity correction and bone transport, hybrid fixators for periarticular injuries, and spanning fixators across joints as part of damage-control orthopaedics in polytrauma patients.

Suitable for open (compound) fractures where contamination makes internal implants hazardous, polytrauma patients requiring rapid fracture stabilisation before definitive surgery, periarticular fractures with severe soft tissue injury, complex deformities requiring gradual correction (Ilizarov), and infected non-unions.

How the Procedure Works

1

Pin/Wire Insertion

Schanz pins (for uniplanar frames) or tensioned fine wires (for ring fixators) are inserted through the skin and bone under fluoroscopic guidance.

2

Frame Assembly

Pins or wires are connected to the modular frame — uniplanar, ring, or hybrid configuration depending on the fracture and indication.

3

Fracture Reduction

Fine adjustments to the frame correct fracture alignment after initial application.

4

Wound Management

Open fracture wounds are irrigated, debrided, and managed — external fixation allows full access to the wound without obstruction.

5

Staged Management

The frame may be converted to internal fixation once soft tissues recover, or maintained as definitive treatment (Ilizarov for lengthening/correction).

Who Needs This Treatment?

  • →

Avoids internal implants in contaminated or open fractures — reduces infection risk

  • →

Rapid application in polytrauma — life-saving damage control

  • →

Allows full access to wound for repeated debridement and dressing

  • →

Adjustable after application — fine-tune alignment in the post-op period

  • →

Ilizarov ring fixator enables bone lengthening and deformity correction

  • →

Spanning fixators provide joint stability while soft tissues recover

"

"External fixation is a versatile, powerful tool — particularly in open fractures and complex reconstructions. The Ilizarov frame, in particular, can achieve things that no internal implant can: growing new bone, correcting deformity, and treating infected non-unions that would otherwise require amputation."

External Fixation (Fracture) Cost in Hyderabad

The cost of External Fixation for fractures in Hyderabad generally ranges from ₹80,000 to ₹1,60,000, with an indicative average cost of around ₹1,20,000. External fixation is a surgical method used to stabilise selected fractures, particularly when internal fixation may not be immediately suitable or when temporary stabilisation is required. The final cost depends on the fracture pattern, soft-tissue condition, type of external fixator, duration of fixation and whether additional procedures are required.

Procedure Approximate Cost Range
External Fixation ₹80,000 – ₹1,60,000

The above range is indicative; the final package and inclusions should be confirmed with the hospital before publication. External fixation may be used as temporary or definitive fracture management depending on the individual injury. In selected cases, further surgery may be required after the soft tissues have recovered or when definitive internal fixation is planned.

Factors Affecting the Cost of External Fixation

The overall cost of external fixation can vary depending on the type and severity of the fracture, soft-tissue injury, fixator configuration, associated injuries and whether the procedure is part of staged fracture management.

  • →

Fracture complexity: The location, displacement, stability and complexity of the fracture can influence the number and configuration of pins, wires or fixation components required.

  • →

Open or severe fractures: External fixation is sometimes used when there is significant soft-tissue damage or an open fracture. Wound management, infection prevention and additional procedures may increase the overall treatment cost.

  • →

Type of external fixator: The configuration and components of the external frame depend on the injury and treatment objective. More complex frames may require additional specialised fixation components.

  • →

Temporary versus definitive treatment: In some patients, external fixation is used as temporary stabilisation before definitive internal fixation. A subsequent operation would generally involve additional costs.

  • →

Associated injuries: Nerve, blood-vessel, muscle, skin, joint or other injuries associated with the fracture may require additional assessment or treatment and can increase the overall treatment cost.

  • →

Hospital and anaesthesia charges: Operating-room charges, anaesthesia, medicines, imaging, surgical consumables and hospital observation may be included according to the hospital's package structure.

  • →

Follow-up and frame management: Regular follow-up, X-rays, pin-site care, frame adjustments or later removal may be required depending on the treatment plan and may involve additional charges.

Insurance Coverage for External Fixation

External Fixation may be covered by health insurance when medically necessary, subject to the patient's policy terms, exclusions, waiting periods, insurer approval and hospital coverage arrangements. Patients should confirm pre-authorisation requirements and applicable coverage with their insurer before treatment.

Dr. Satish Reddy Gandavarapu

Orthopaedic & Joint Replacement Surgeon

Dr. Satish Reddy at Lux Hospitals, Hyderabad, can evaluate patients with complex fractures and traumatic injuries and determine whether external fixation is appropriate based on the fracture pattern, soft-tissue condition and overall clinical situation. During the consultation, the doctor may assess: Injury details: The doctor may review how the injury occurred, the timing of the trauma and the severity of pain, swelling, deformity or functional limitation. Fracture pattern: X-rays and other imaging may be reviewed to determine the location, displacement, stability and complexity of the fracture. Soft-tissue condition: The condition of the skin, muscles and surrounding soft tissues may be assessed, particularly when there is significant swelling, wound contamination or an open fracture. Circulation and nerve function: Blood supply, sensation, movement and other neurovascular findings may be evaluated to identify associated injuries requiring urgent attention. Fixation suitability: The doctor may determine whether external fixation is appropriate as temporary or definitive stabilisation and whether additional procedures may be required later. Follow-up planning: The doctor may explain pin-site care, weight-bearing or activity restrictions, follow-up imaging and the expected pathway for frame management or subsequent definitive treatment.
