Capsular Shift / Capsular Plication
Surgical tightening of a loose shoulder capsule to restore joint stability, prevent recurrent subluxations, and improve shoulder control — particularly for multidirectional instability.
What is Capsular Shift / Capsular Plication?
Capsular Shift or Capsular Plication surgically tightens a loose or overstretched joint capsule — the soft-tissue envelope that holds the humeral head within the glenoid socket. When the capsule becomes lax due to repeated dislocations, sports injuries, or generalised ligament laxity, the shoulder may subluxate (partially slip) or dislocate in multiple directions. The procedure reinforces the capsule either by folding and suturing it inward (plication) or by shifting and repositioning it (shift), restoring the normal tension needed for a stable joint. It is the treatment of choice for multidirectional shoulder instability when physiotherapy alone has not provided sufficient improvement.
SUITABLE FOR Patients with multidirectional shoulder instability or a chronically lax joint capsule causing repeated subluxations or dislocations — where physiotherapy has been inadequate and surgical capsular tightening is required.
How the Procedure Works
Pre-operative MRI Assessment
MRI evaluates capsular laxity, labral integrity, and the direction and severity of instability; the surgical plan is tailored accordingly
Anaesthesia & Arthroscopic Access
Regional or general anaesthesia is administered; small keyhole incisions allow the arthroscope to fully visualise the capsule and surrounding joint structures.
Capsule & Joint Evaluation
The surgeon assesses capsular volume, laxity, and any associated labral, ligament, or chondral injuries within the shoulder joint.
Capsular Tightening
Using arthroscopic instruments, the loose capsular tissue is gathered or folded; sutures reduce capsular volume and restore the correct degree of joint tension.
Shift or Plication
Depending on the instability pattern, a capsular shift (repositioning) or plication (folding) is performed to achieve balanced, multi-directional stability.
Stability Check & Wound Closure
Joint stability in all planes is confirmed; instruments are removed, incisions are closed, and the arm is supported in a sling.

Capsular Shift / Plication Cost in Hyderabad
The cost of Capsular Shift / Plication in Hyderabad generally ranges from ₹1,00,000 to ₹2,00,000, with an indicative average cost of around ₹1,50,000. Capsular shift or plication is a shoulder stabilisation procedure that may be considered for selected patients with shoulder instability, particularly when excessive capsular laxity contributes to recurrent or multidirectional instability. The final cost depends on the severity and pattern of instability, surgical technique, implant requirements, associated procedures and postoperative rehabilitation.
| Procedure | Approximate Cost Range |
|---|---|
| Capsular Shift / Plication | ₹1,00,000 – ₹2,00,000 |
The above range is indicative; the final package and inclusions should be confirmed with the hospital before publication. The appropriate surgical approach depends on the type and severity of shoulder instability, examination findings, imaging and the condition of the surrounding shoulder structures.
Factors Affecting the Cost of Capsular Shift / Plication
The overall cost of capsular shift or plication can vary depending on the pattern of instability, degree of capsular laxity, surgical approach, fixation requirements, associated shoulder conditions and postoperative rehabilitation needs.
- →
Type and severity of instability: Multidirectional instability, recurrent dislocation or persistent shoulder laxity may require different levels of surgical correction and treatment planning.
- →
Surgical technique: Capsular shift or plication may be performed using arthroscopic or other surgical approaches depending on the patient's condition and the surgeon's assessment.
- →
Suture anchors and implants: Arthroscopic stabilisation may require suture anchors or other fixation devices, and the number and type of implants can affect the overall cost.
- →
Associated shoulder conditions: Labral tears, rotator cuff problems, cartilage damage or other structural abnormalities may require additional treatment during the same procedure and can increase the final cost.
- →
Preoperative investigations: X-rays, MRI scans, blood tests and other investigations may be recommended to assess instability and prepare for surgery. These may be billed separately when not included in the package.
- →
Anaesthesia and hospitalisation: Anaesthesia, operating-room use, nursing care, medicines and hospitalisation may contribute to the overall treatment cost according to the hospital's package structure.
- →
Postoperative rehabilitation: Physiotherapy and a structured rehabilitation programme are generally important after shoulder stabilisation surgery. Rehabilitation costs may be separate from the surgical package.
Insurance Coverage for Capsular Shift / Plication
Capsular Shift / Plication may be covered by health insurance when medically necessary, subject to the patient's policy terms, exclusions, waiting periods, insurer approval and hospital coverage arrangements. Patients should confirm pre-authorisation requirements and applicable coverage with their insurer before treatment.

Dr. Satish Reddy Gandavarapu
Orthopaedic & Joint Replacement Surgeon
Dr. Satish Reddy at Lux Hospitals, Hyderabad, can evaluate patients with recurrent or multidirectional shoulder instability and help determine whether capsular shift or plication is appropriate based on clinical examination, imaging and the overall condition of the shoulder. During the consultation, the doctor may assess: Instability symptoms: The doctor may review episodes of dislocation or subluxation, a feeling of the shoulder slipping, pain, weakness and difficulty with sports or overhead activities. Shoulder examination: Range of motion, joint stability, ligamentous laxity, muscle strength and apprehension during specific shoulder movements may be evaluated. Pattern of instability: The direction and frequency of instability episodes may be assessed to determine whether the problem is primarily anterior, posterior, multidirectional or related to another structural abnormality. Imaging findings: X-rays and, when indicated, MRI or other imaging may be reviewed to evaluate the labrum, cartilage, rotator cuff, bone structure and other shoulder tissues. Previous treatment: Physiotherapy, activity modification, bracing, medications and previous stabilisation procedures may be discussed along with the response to treatment. Surgical suitability: Based on the overall findings, the doctor may discuss capsular shift or plication and other appropriate treatment options for stabilising the shoulder.
